To request a speaker from the Office of Equal Opportunity, Equity & Affirmative Action in a classroom, program, project, or meeting, please turn in this completed form to the Director of EOEAA, at the UW Oshkosh campus (Dempsey Hall #328), or email your request to afaction@jaatisekunn.net. Upon receipt, EOEAA will review all documentation and reply to requestors with a decision regarding proposed level of involvement within 5 business days.
